2024 Credentials Application Information

The 2024 ACVS Credentials Application Guidelines are available for all current and former residents wishing to submit a credentials application. Applicants should download and review the Guidelines which outline the process and requirements for submitting an application.

First time credentials applications must be submitted online via the Applications button in CERT on or before 11:59 pm (Eastern Daylight Time) on August 1, 2024

Individuals reapplying following three unsuccessful examination attempts must submit the Credentials Reapplication form on or before 11:59 pm (Eastern Daylight Time) on August 1, 2024.

The application fee is $1,225. The fee must be paid online on or before the deadline. Payment of the application fee is the final step of the submission process. Applications are not considered submitted until the fee has been paid.

IMPORTANT: Submitting your credentials application is separate from examination registration. Should an individual’s credentials or recredentialing application not be accepted by ACVS, the individual will be responsible for canceling their appointment with Pearson VUE. The deadline to register for the Phase II examination is July 22, 2024, 11:59 pm (Eastern Daylight Time). See the Phase II Examination page for more information.

Applicants who performed surgeries at locations outside of your main program location and entered into your logs prior to the transition to CERT must submit an ACVS External Surgical Rotation Form with all relevant signatures. Please see the Credentials Application Guidelines for information on how to submit this form.
