Chapter 4 - Credentials Application


The 2010 Credentials Application packet (Credentials Application Guidelines and Preparation of ACVS Credentials Applications) will be available from this section of the ACVS website by mid-March. All residents wishing to submit an application must use the 2010 Forms, regardless of when the residnecy program began. The application packet will have instructions for resident users of the online Resident Training Log system. 2010 will be the first year that residents who have used this system are submitting credentials applications.

It is imperative that residents add all cases, activity weeks and seminars, as well as specialty service rotations, to the online logs and submit them for Resident Advisor/Supervisor verification in a timely manner. Program Directors, Resident Advisors and superivsors of specialty service rotations must complete their reviews on the website prior to the resident submitting the application packets to the ACVS office.

Residents must print out certain items from the website to include in the copies of the credentials application packets, which must arrive at the ACVS office on or before August 2. Instructions for applicants will be in the Credentials Application Guidelines and Preparation of ACVS Credentials Applications documents.  A separate document will also be available specifically for ACVS Diplomates serving as Program Directors and Resident Advisors.  That document will be included on this page along with the application materials.
