| 2004 ACVS Information Brochure |
The ACVS Information Brochure contains information about membership requirements, credentials, residency programs, "Diplomate" use guidelines, and more. As of July 7, 2004, the Residency Program Documentation files are not yet available to be downloaded. Residents should consult the printed copy of the 2004 Information Brochure (mailed 07/06/2004 to all current residents) for complete guidelines and requirements.
The entire 2004 Information Brochure can also be downloaded in rtf format. The required forms can be downloaded from the links below.
For Residents beginning programs between July 1, 2004 and June 30, 2005, all program data must be maintained using the electronic ACVS Residency Program Documentation file. This Microsoft Excel workbook file will be available from the ACVS web site (www.acvs.org) or from the ACVS office. Residents must have access to Microsoft Excel in order to use this file. There are three versions of this file, one for each Core Curriculum (i.e., LA-Equine, LA-General, Small Animal). All of the forms required for annual submission are contained in this Excel workbook file as individual worksheets. Annual submission of this Excel file will be required. The Excel file and Information Brochure will include detailed guidance for using this Excel file.
Note: Residents who began their programs between July 1, 2000 and June 30, 2002, have the option to use the new Residency Program Documentation file. However, they must still comply with the requirements of the Information Brochure in effect when they started their programs. Residents who choose this option should download and read these special instructions. Residents who began their programs between July 1, 2002 and June 30, 2004, may migrate their data to the 2004 Excel version, but they are not required to do so.
