The ACVS Information Brochure contains information about membership requirements, credentials, residency programs, "Diplomate" use guidelines, and more. As of July 20, 2001, the forms for submitting residency logs and credentials applications are available to be downloaded. Residents should consult the printed copy of the 2001 Information Brochure (mailed 7/9/01 to all current residents) for guidelines and checklists for completing the forms. The entire 2001 Information Brochure can also be downloaded in rtf format.

Residents who started their programs prior to July 1, 2001, were given the opportunity to switch to follow the guidelines in the 2001 brochure and submit their program documentation using the 2001 forms.
