It is never easy to lose a patient and the ACVS Foundation recognizes how difficult this time is both for you and your client. Participating in the Pet and Equine Memorial Program is a meaningful way to communicate your support and concern to your client in a simple and rewarding way.

The ACVS Foundation will personalize a high quality memorial card and mail the card to your client within seven business days of receiving the client's information. At your discretion, the Foundation can also notify the referring veterinarian of your gift. The card will never show the amount of your donation. To participate using this option, you can send the information to the ACVS Foundation office online.  If you prefer to mail or fax your donation, you can download and submit the Pet and Equine Memorial Programs Donation Form.

Your contribution to the ACVS Foundation's Memorial Program provides valuable support for the Foundation’s good work toward better surgical and medical care for all animals, and is tax deductible to the extend allow by the Internal Revenue Service.

The ACVS Foundation appreciates the support from the practices that are currently participating in the Pet and Equine Memorial Program. This list reflects practices that have participated in 2011, and one or more Diplomates within the practice may be making memorial donations as well. We thank each practice and individual for assisting the ACVS Foundation in supporting the research and educational mission of the College!
